HOW CAN OUR ORGANIZATION ORDER THE HPN PROGRAM

HEALTHY LIVING:  A Health Risk Appraisal for Older Adults  (HRA-OA).  The Participant fills out a printed questionnaire, which is then processed by the HPN computer program to generate a printed report.  The questionnaire, printed in 14-point type, consists of the 43 questions of the Midlife Adult HRA, plus additional questions for persons ages 55-90.  Whereas the calculations for the Midlife questions are based on mortality statistics, the added questions for the HRA-OA focus on functional status and morbidity.  These questions are divided into eight modules:

The individualized report consists of the standard mortality based report for the Midlife HRA questions plus a report for the following outcomes:

   Risk for Institutionalization

   Functional Status

   Physical Disability

   Falls

   Vision Impairment

   Hearing Impairment

   Nutrition

   Adverse Drug Reactions

   Influenza/Pneumonia

   Mental Health

   Home Environment Hazards

   Burns and Fire Related Injury

   Tetanus

   Existing Conditions

   Social Supports

The outcomes, precursors, and questions appropriate for the older adult population were selected after an extensive literature review.   A draft of the questionnaire was field tested in five sites with various populations in Arizona, Georgia, Iowa, Tennessee, and Washington for a total of 1,400 Participants.  Reports from the field test administrators indicate that the HRA-OA was well received by the Participants, most of whom completed the questionnaire in less than an hour. The results of the field test reports were compiled and analyzed to arrive at norms, which are used to develop the morbidity and functional status estimates and report modules. The HPN Supporting Materials also provide the option for the User to print the Age Pages produced by the National Institute on Aging.  HPN provides the User with a list of the Age Pages so that the Participant can select those of interest.
